How Do I.... Register With The Practice?

Each person registering with the Practice will be asked to complete a GMS1 Registration Form, which is available from reception.
Each newly registered patient will be added to the Practice list.
Patients will also be given a questionnaire to complete in which they have the right to express a preference to be seen by a particular Doctor.
It is Practice policy, as part of the registration procedure, for all new patients aged five years and over to be offered an appointment with the nurse for a new patient health check. This includes checking blood pressure, testing urine, measuring height and weight and giving advice on a healthy lifestyle. The nurse will also note any current problems or allergies on your medical history.
